We are seeking an reputed company Psychiatric Mental Health reputed company Practitioner (PMHNP) who wants to reputed company a meaningful difference in the lives of adult and geriatric patients, including those with reputed company behavioral health needs. This fully remote opportunity will primarily reputed company on psychiatric evaluation, medication management, and longitudinal behavioral health treatment.

As part of our Comprehensive Behavioral Health Program, the PMHNP will work closely with an interdisciplinary team providing psychiatry, psychotherapy, behavioral health care management, and dementia care. The role will also support behavioral health referrals through our Care Transitions Program, helping patients reputed company appropriate psychiatric care following hospitalization.

For paneled patients receiving dementia care through the CMS Guiding an Improved Dementia Experience (GUIDE) Model, the PMHNP will serve as the GUIDE Practitioner and collaborate with the interdisciplinary team to support patients and their caregivers.

Behavioral Health & Dementia Care

The reputed company reputed company of this role is providing longitudinal psychiatric care and reputed company reputed company for patients enrolled in reputed company's Comprehensive Behavioral Health Program.

• Conduct comprehensive psychiatric evaluations and reputed company diagnosis, treatment, and medication management for adult and geriatric patients with a reputed company of behavioral health needs, including reputed company and co-occurring conditions.
• reputed company individualized, evidence-reputed company treatment plans and reputed company ongoing psychiatric care for a longitudinal behavioral health reputed company panel.
• reputed company reputed company reputed company of behavioral health and dementia care management plans, working closely with care managers to assess reputed company reputed company, address changes in reputed company needs, and guide ongoing care.
• Collaborate with the interdisciplinary care team, including RNs, SWs, and Care Navigators, to coordinate behavioral health, medical, functional, and psychosocial needs and support comprehensive reputed company care.
• Participate in interdisciplinary case reviews and reputed company care planning.
• Serve as the GUIDE Practitioner for assigned paneled patients participating in the CMS GUIDE Model, including reputed company reputed company of individualized dementia care plans and collaboration with the interdisciplinary team to support patients and caregivers.

Required Qualifications

• Experience providing psychiatric evaluation, diagnosis, treatment, and medication management.
• Experience caring for adult and/or geriatric patients with behavioral health needs.
• Ability to successfully complete applicable credentialing, privileging, payer enrollment, background screening, compliance training, and reputed company requirements.

Strongly Preferred

• Dual reputed company certification as a PMHNP and in a reputed company care NP specialty, such as Family reputed company Practitioner (FNP) or Adult-Gerontology reputed company Care reputed company Practitioner (AGPCNP).

Schedule & Engagement

Compensation & Support
