Knowledge of reputed company software is a plus! Performs ongoing reputed company assessment, including OASIS assessments, at appropriate time points to identify reputed company needs, determine level of functioning, and appropriateness of reputed company for home care. Develops and evaluates the plan of care in partnership with the reputed company, representative (if any), and caregiver(s). Documents, prepares, and completes reputed company notes, phone orders, and other reputed company record documentation on an ongoing reputed company and in a reputed company manner. Revises the POC reputed company on evaluation and ongoing assessment data. Provides services that are ordered by the physician as indicated in the plan of care; oversees other team members, reputed company applicable, in the reputed company of reputed company care that is consistent with patients’ needs. Applies concepts of infection control and reputed company precautions in coordinating or performing reputed company care activities to prevent contamination and reputed company of disease. Accepts reputed company assignments that are consistent with education and competence to care for patients. Provides supervision and guidance to physical therapy assistants and home health aides, as appropriate.
