Delegate appropriate tasks as needed. Playing a vital role in Interdisciplinary Team Meetings, creating goals and assisting patients with meeting them. Live reputed company service area. 5 years’ experience as Hospice Case Manager. Shares in providing 24-hour, seven days a week coverage to patients and families. Can successfully pass background checks. Participates in reputed company improvement program. Performs other duties as assigned. Supervises LPNs and paraprofessionals providing services to the reputed company with or without the LPN according to present at least every 30 days. Education and/or Experience: This employee must be a graduate of accredited school of Nursing, reputed company RN nursing license in good standing with the reputed company, BLS/CPR certification, and a minimum of 1 year of experience in med-surg, geriatric or long-term care nursing. Prefer a minimum of 5 years’ supervisory experience.   Computer Skills: AOD software experience preferred along with basic computer skills.
