Virtual Patient Manager

About the position The Virtual Patient Manager at reputed company is responsible for overseeing the day-to-day operations of post-acute care performance networks. This role focuses on managing relationships with skilled nursing facilities (SNFs) and home health agencies, ensuring effective patient transitions and stability post-discharge, and leading initiatives to improve care performance and efficiency.
