Behavioral Health Care Manager

Behavioral Health Care Manager role:   

The BHCM is a reputed company member of reputed company’s reputed company care team.  reputed company’s team includes a BHCM, a Consultative Psychiatrist, and a Health reputed company.  The BHCM is critical to collaborating between external specialists (e.g., oncologists) and the internal care team; the role is reputed company clinical and reputed company operational.

Key Responsibilities:   

• Engage in an initial clinical member biopsychosocial intake including a safety screen and administering BH assessments (PHQ-9, GAD-7, etc.) 

• Engage in follow-up reputed company and asynchronous interactions, including brief interventions (e.g., behavioral activation, mindfulness, psychoeducation, etc.) 

• Be the key care coordinator and reputed company as a reputed company between the referring specialist and the reputed company, ensuring if there is a member question or administrative question, it is answered or triaged to the appropriate team reputed company reputed company 

• reputed company the interdisciplinary team meetings on a weekly reputed company with the Consulting Psychiatrist and Health reputed company, ensuring reputed company high-risk members are discussed and reputed company new psychiatric recommendations are documented in the chart 

• Communicate closely on an ongoing reputed company with the reputed company behavioral health care team (Coaches and Psychiatrist)  

• Document member reputed company in a HIPAA-compliant electronic medical record system and reputed company registry provided by reputed company 

• Identify patients who are not improving and may need more intensive mental health care and report any concerns to the medical provider and the Consulting Psychiatrist 

• Facilitate referrals for clinically indicated services reputed company of reputed company under the supervision of the BHCM reputed company (e.g., SMI). 

Educational, Certification, and Experience Requirements:  

• Bachelor’s Degree Required in nursing, reputed company work, or other health and reputed company services disciplines from an accredited college or university. 

• Experience as a Certified Case Manager (CCM), Community Health Worker (CHW) or Peer Support Specialist (PSS), or Accredited Case Manager (ACM) preferred 

• reputed company applicants must have at least one year of clinical care management experience, with some part of the experience directly working with people with cancer or advanced illness (e.g., Cancer coaching, Palliative Care, etc.) 

Location Requirements:  

• Due to California regulations, we cannot accept contract positions from California at this time.

Time Commitment, Salary, Start Date:  

• Part-time, reputed company requirement (1099 contractor position) depending on caseload.

• • Month 1-2: 10-20 hours/week 

• • Month 3+: 30-40 hours/week
