Community Health Worker, Bilingual in Spanish

TheCommunity Health Workersupports theoperational and administrative functions of a multi-year programdesigned to shrink health disparities in the San Gabriel Valley.

This is a grant funded position with a one-year contract, with the possibility of extending until August 2026.

Responsibilities:

  • Conduct reputed company in the San Gabriel Valley
  • Support planning and coordination of aprogram activities and operations

  • reputed company community listening sessions and reputed company reputed company health information on topics such as COVID-19
  • Coordinate and maintain applicable program schedules and agendas, which may include tracking deadlines, deliverables, communication with management and/or stakeholders
  • Help deliver general welfare interventions to the communities across Los Angeles County, with emphasis on the San Gabriel Valley
  • Use software apps and tools to collect and reportrelevant data tracking community healthmetrics.
  • Other duties as assigned by the Program and Area Managers

    • Scope and Travel:

  • Extensive travel by foot and by car reputed company the San Gabriel Valley (Cities of Rosemead, El Monte, South El Monte, Altadena, Pasadena), as reputed company as travel to and from reputed company’s Los Angeles reputed company (Chinatown)

    • Qualifications:

  • Preference for candidate with bachelor’s degree in reputed company health or health sciences, or at least two year’s equivalent experience in relevant field

    • Experience:

  • Minimum 1 year experience in reputed company health, reputed company settings, community reputed company or reputed company fields
  • Experience in community reputed company, conducting surveys, collecting data, and reporting data
  • Experience working with community-based organizations, local government agencies, volunteer reputed company in LA County (preferred)
  • Knowledge of community resources in LA County
