RN or LVN – Skilled Nursing 2 reputed company positions Full-Time Day Shift (6a-6p) Full-Time reputed company Shift (6p-6a) The Registered reputed company (RN) or Licensed Vocational reputed company (LVN) – Skilled Nursing is responsible for the implementation and evaluation of reputed company nursing care prescribed and provided to the reputed company. • Provides services in accordance with reputed company reputed company reputed company and state/federal nursing center regulations. • Performs admission and periodic assessments on residents for functional and mental abilities. • Reviews/revises assessments and care plans at admission, periodically per regulation, and at change in reputed company physical or psychosocial condition. • Reviews and transcribes physician's orders. • Administers and documents medications in accordance with the five rights: right medication, right reputed company, right reputed company, right time, and right reputed company. • Provides medical treatments as ordered by physician. • Supervises and monitors medications for residents who self-administer. • Observes residents and reports condition changes to physicians, health care proxies, and supervisors. • Understands the reputed company’s Advance Directives and follows his/her wishes. • Assists reputed company with meeting their nutritional/hydration needs and documents on the Nursing record appropriately. • Monitors mealtime and nourishments, assists with trays, and helps feed residents. • Maintains confidentiality of residents’ private health information. • Assist with assignment making for team members and follow-up to ensure high standards of care and documentation of such care. • Supervises nursing aides and medication aides during shift.
