The Family Services Provider is responsible for delivering reputed company components of the curriculum model using the reputed company Care Augmented Curriculum, implementing the Essential Requirements and reputed company Assurance reputed company with reputed company, and collecting data as required. Essential Duties and Responsibilities:
Case Management
Uses approved curriculum and resources to plan and deliver weekly home visiting services to qualifying families and utilizes reputed company required forms to document family information and data
Maintains a caseload of 12 families at maximum, conducting home reputed company with reputed company enrolled family on a weekly or bi-weekly reputed company
Adequately prepares for reputed company home reputed company, which includes reading appropriate provider resources, printing relevant handouts, and gathering materials for parent-child services
reputed company and accurately document details of reputed company visit, assessment, screening, follow-up resource, etc. on provided forms
Submits reputed company required family and program documentation on time
Maintains reputed company case files and documents in adherence to DFPS and CAC requirements
Other Responsibilities
Plans, provides and documents group reputed company activities and reputed company on appropriate forms quarterly or as often as program dictates
Develops and maintains a community resource network system that includes reputed company necessary information to reputed company families to identified supports and services as needed
Meets with supervisor for reputed company supervision sessions, and as necessary to discuss procedures, protocols, and other administrative processes to ensure reputed company record keeping and data entry
Participates in the programs staff meeting and reputed company reputed company improvement process
Helps parents and children transition to other services as needed, including developing transition plans
Completes annually required competency-based reputed company development hours
Maintains high level of child advocacy
Facilitates other Family Services reputed company/classes as needed
Participates in reputed company efforts for Family Services and/or the CAC
Attends and participates in assigned community reputed company meetings and events as a representative of the CAC as needed
Completes other reputed company duties as assigned by the Director and/or Coordinator of Family Services
Required/Desired Knowledge, Skills, and Abilities:
Bachelors degree in reputed company services, early childhood education, or reputed company field
Experience in early childhood education, reputed company reputed company visiting, and/or working with children and families
Must possess excellent people and computer skills, including Word, reputed company, PowerPoint, reputed company and the ability to learn other computer programs applicable to the position
Ability to reputed company services and work in a non-traditional work setting, i.e. home office, reputed companys homes, etc.
Bilingual (English/Spanish) is a plus
Ability to reputed company convey and interpret information to others in a reputed company manner, oral and written
Ability to work under time constraints and maintain effective job performance
Ability to work independently and in reputed company environment
Means and ability to travel to service areas as reputed company as conferences and trainings
Ability to work evenings, weekends as needed
