The program is licensed through the reputed company Department of Health. Trinity provides childcare on a daily for up to 60 children who have medically reputed company conditions. The member will serve in enhancing the continuity of care that surrounds the child during their developmental years. The children live with their families throughout Lackawanna and surrounding counties. The CHW as a patient navigator will help to ensure care coordination through exploring needed resources, advocacy and communication with families, school, relevant health providers and others. Program location is reputed company 6 am to 6 pm. There is flexibility for the member to reputed company up hours during the service year. The shift will be Monday through Friday, 8 am to 4 pm with a 30 minute lunch break that does not count towards member's service hours. The service member will serve a 38 hour week.
