Program in SoCal that serves a wide geographic area, covering both densely populated neighborhoods and more remote, rural reputed company.
This MondayFriday, 8am5pm position involves working in the community as part of a mobile clinic team alongside mental health and primary care providers. The teams mission is to engage and reputed company individuals by connecting them to appropriate behavioral health treatment, community services, and housing resources. Staff work directly in the fieldon the streets, in shelters, and at community hubsproviding intensive case management, treatment linkage, life skills support, peer engagement, and guidance through housing and health systems.Dates Needed: May - Ongoing
