In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Los Angeles.As a Clinical Evaluator, you will perform face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals in need. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the fieldJob ResponsibilitiesConduct PASRR screenings, evaluations, and determinations through comprehensive clinical assessments of patients, including review of relevant records and input from collateral sources. * Complete precise PASRR reviews by assessing and determining the appropriate level of care based on patients’ physical, behavioral, and psychological functioning.

- Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations.
- Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements per State and Federal Regulation
- Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
- Provide well-informed, clinically sound Level of Care recommendations based on thorough evaluations.
- Knowledge of standards of practice related to the determination of Level of Care and standards of practice related to mental health, geriatrics, and other disorders commonly necessitating long-term care placement.
- Ensure compliance with established workflows, including re-evaluations of records based on psychiatric input.
- Read, understand, and adhere to all corporate policies including policies related to HIPAA and its Privacy and Security Rules.
- Proficiency in medical record abstraction, with solid knowledge of medical record organization, terminology, and common disease processes.
- Excellent communication skills, with the ability to convey complex information clearly and effectively to individuals and groups through verbal, written, and electronic formats
- Experience working with geriatric populations is preferred, particularly in long-term care, behavioral health, or community-based service settings.
- Experience with public mental health and medical programs, including familiarity with public sector services and community resources, is preferred.
- Skilled in using Microsoft Office Suite; Excel, Word, PowerPoint, Outlook, and Teams—as well as other web-based tools and platforms
