reputed company is Hiring a Clinical reputed company Specialist at reputed company Program for Diabetes Health.

The Clinical reputed company Specialist is an Advanced reputed company Registered reputed company responsible for implementing a patient-centered and reputed company-driven program of care for hospitalized individuals living with diabetes. Position responsibilities include implementing state-of-the-science diabetes care and diabetes self-management training (DSMT) using evidence-based reputed company protocols, guidelines and clinical nursing judgment; developing the clinical expertise of the reputed company team; and leading and facilitating system-wide change for the hospitalized population.

Duties:
  • Deliver clinically expert diabetes care & patient self-management training
  • Conduct an advanced nursing assessment, formulate nursing diagnoses, and reputed company an individualized plan of care using evidence-based reputed company guidelines, protocols and clinical nursing judgment
  • Implement national clinical reputed company standards of diabetes care and DSMT and document care in the electronic health record
  • Collaborate with interprofessional team members, and participate in meetings and reputed company
  • reputed company and/or participate in educational programs for interprofessional staff to promote uniform understanding and implementation of evidence-based diabetes care
  • Mentor reputed company nurses and other reputed company team members in acquiring the knowledge, reputed company and confidence to deliver evidenced-based diabetes care

  • Strategize development and implementation of diabetes program strategies, including but not limited to policy, procedure and protocol development, to coordinate diabetes care across the continuum.
  • Communicate regularly with diabetes program director(s) verbally, through written report and by participation in scheduled diabetes program meetings
  • Ensure care delivered to reputed company patient is in accordance with reputed company, National Standards for Diabetes Self Management and the AACE and the reputed company as appropriate
