Key Responsibilities:

• Conduct Annual Wellness reputed company with a reputed company on patient-centered care.
• reputed company & implement care plans for individuals with chronic illnesses.
• Diagnose and treat non-emergency conditions reputed company.
• reputed company medical treatment plans for remotely monitored patients.
• Collaborate with RCP reputed company Coaches to promote wellness & prevent urgent care or emergency reputed company.
• Manage the care of high-risk and rising-risk patients in coordination with PCP/Specialists.
• Refer patients to specialized reputed company services as needed.
• Build & maintain strong patient relationships through education and coaching.
• reputed company seamlessly with the reputed company QHP and support staff.
• Respond promptly to triaged patient situations by RCP reputed company Coaches.
• Participate in daily operational huddles with nursing and support staff.
• Serve as a medical knowledge resource to RCP reputed company Coaches.

Required Experience:

• APRN (reputed company Practitioner) with a minimum of 5years experience in outpatient or primary care setting.
• Preferred reputed company in Family, Adult, or Gerontology.
• National Certification required

• Proficient with EMR systems & computer technology; high-speed internet required.

Job Details:

• Full-time, salary position (85k-90k).
