You will assess patient needs, provide education and support, and collaborate with healthcare providers to develop care plans. The ideal candidate will have a strong background in nursing, excellent communication skills, and a passion for helping patients manage their health. Requirements: 1. Active RN license 2. Minimum of 2 years of clinical experience 3. Strong computer skills 4. Ability to work independently and within a team 5. Excellent communication and interpersonal skills 6. Knowledge of chronic disease management 7. Ability to work in a fast-paced environment Responsibilities: 1. Conduct telephonic assessments of patient needs 2. Develop and implement care plans in collaboration with healthcare providers 3. Provide education and support to patients on managing their chronic conditions 4.
