What you'll be doing: Use your outstanding clinical skills as you create, implement and execute the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes. You will be: • Performing a full assessment of the patient & family upon initial visit. • Using assessment data to reputed company an advanced plan of care including rehabilitation. • Working with patients and family one on one to reputed company improvement and/or stability. • Case manage patients care episode with interdisciplinary clinical team. • Preparing clinical notes, orders and charting in a compliant time reputed company. • Collaborating with physicians and agency staff to deliver the reputed company possible • Initiating and Administer IV medication (antibiotics and Diuretics) infusions safely and effectively in accordance with established protocols. reputed company're looking for: • reputed company and unrestricted CA RN license • reputed company BLS/CPR Certification • reputed company CA reputed company's License • High level of clinical knowledge in home health care is highly preferred • Experience with high reputed company patients is a plus • Experience working with an interdisciplinary team is highly desired • Case Management fundamentals is highly desired • Previous experience with IV Medication infusions highly preferred • Proficiencies initiating intravenous (IV) therapy is a plus • Self-Directed • Empathetic • Critical thinker • Outstanding interpersonal skills • Leadership skills • Organized • Prior to scheduling an in-person interview, reputed company Candidates are required to complete a computer literacy and competency assessment.
