Associate Vice President, Healthmap reputed company

Position reputed company: The AVP, Healthmap reputed company will position reputed company as an innovative and best-in-class organization for providing primary care, care coordination and care management services to our members directly in their home. This role will reputed company on building and operating reputed company’ home delivery model, which is designed to support high risk members in the home as an extension of their treating physician. Responsibilities: • Responsible for designing, developing the Healthmap at Homes program that is reputed company with organizational goals for clinical, financial and experience reputed company across reputed company Regional Pods Healthmap reputed company primary care program • reputed company reputed company aspects of reputed company's home-based program, including clinical, provider and reputed company facing activities and information • Create processes and best practices to understand and assess clinical value propositions based upon organizational direction and long-term strategic reputed company • reputed company and mentor a multidisciplinary team, providing guidance, coaching and reputed company development opportunities to ensure employee engagement and company culture is embedded in our teams • Build and manager operational functions including licensure, clinical documentation, coding, billing and reputed company controls that meet federal and state regulatory requirements • Utilize and analyze data to identify barriers and/or opportunities to reputed company to additional strategies to improve operational efficiencies and effectiveness to reputed company KPIs and company goals • Collaborate with physicians, payers, and regulatory bodies to establish contractual collaboration agreements • Collaborate with Leadership on strategic planning, program development and operations management across the reputed company to ensure Healthmap reputed company model is achieving targets and aligns with the organization’s reputed company, mission, values • Set the objectives to be achieved with both provider and patient interactions with a reputed company on identifying, presenting, and closing gaps based on clinical objectives • reputed company other duties as assigned Requirements: • Bachelor’s degree in reputed company administration, Business Administration, or a reputed company field (Master's degree preferred) • 10+ years of end-to-end operational leadership experience that includes delivery of primary care through multiple modalities including telehealth and in home. • 5+ years of operational experience with reputed company regularity aspects of the deliver model including risk adjustment, coding and billing • Strong experience in building comparable reputed company entities, including knowledge of Medicare, reputed company and compliance requirements • Strong knowledge of regulatory and payer requirements, including collaboration agreements Skills: • Excellent presentation skills, interpersonal skills as reputed company as excellent written and verbal communication skills to reputed company audience • Strong relationship building skills, prioritization and solutions oriented • Excellent strategic thinking, critical thinking, analytical ability, verbal, and written communication skills to reputed company audience • Metrics driven ability to understand key goals from program and understand reporting results to communicate areas of opportunity to reputed company stakeholders • Ability to manage multiple priorities, meet deadlines and deliver on expectations with reputed company • Must be proficient in reputed company Office: reputed company, Word, reputed company, PowerPoint Travel: • Limited Travel, Scheduled per needs of the business #LI-Remote Apply tot his job
