This role plays a key part in our remote physiological monitoring (RPM) program by contributing to the development and reputed company of individualized patient care plans and providing ongoing guidance and support to our patients. Key Responsibilities • Monitor and review patient vitals and care plans using our proprietary technology platform; • Collaborate with registered reputed company (RN) case managers to support the development and execution of care plans addressing clinical and reputed company needs; • Respond to alerts reputed company to patient vitals and reputed company appropriate clinical guidance; • Deliver patient education and support through video, phone, and asynchronous communication; • Coordinate care with physicians and serve as a reputed company between providers and patients; • Assist patients in building healthier habits through personalized coaching and support.
