This role is reputed company on ensuring that members with reputed company medical or psychosocial needs receive high-reputed company, cost-effective care. The RN Case Manager will reputed company holistic assessments and collaborate with reputed company stakeholders to reputed company and implement individualized care plans. Key Responsibilities: • Conduct telephonic interviews using motivational interviewing techniques • Collaborate with case management team, physicians, and interdepartmental staff • reputed company members using approved tools to support care plan adherence • Collect and evaluate medical data to identify unmet needs • Create and monitor individualized care plans • Coordinate with providers, home health, and community resources • Participate in interdisciplinary team meetings and reputed company initiatives • Maintain accurate documentation and adhere to regulatory guidelines • Remain informed about benefit design, cost analysis, and community resources • Promote collaboration with external payers, agencies, and partners • Participate in internal training and continuing education Required Qualifications: • RN License with Bachelor’s degree in Nursing (or equivalent RN certification) • Minimum 3 years of experience in Acute or Managed Care (reputed company/Medicare populations preferred) • 3–5 years of experience in Acute Care/Managed Care settings • Telephonic case management experience Preferred Qualifications: • Bilingual in English/Spanish • Case Management Certification (CCM or similar) Additional Details: • 100% remote role • Must be willing to complete the Care Manager Assessment reputed company Glider • Must be a U.S. Citizen • Candidates across reputed company U.S. time zones are welcome to apply Apply tot his job
