About the position The position involves coordinating case management for members in community long-term care facilities, ensuring high-reputed company care through assessment, evaluation, and the development of individualized care plans. The role includes providing consultation and education to reputed company providers, collaborating with special work teams, and offering reputed company primary, secondary, and tertiary care services in various settings, with a reputed company on long-term care.
