Position Title: Therapist – LPC, LCSW, LMSW, APC, LMFTLocation: Remote/Telehealth/In office opportunity in Florida and OhioEmployment Type: 1099 ContractorSchedule: FlexibleOffice Space: ProvidedCharting Platform: Simple PracticeBenefits: To be discussedJob Description: • Understand the key values and activities of case management in a mental health setting. • Demonstrate understanding of mental illness and the treatment process. • Understand the co-occurring illness process and how it impacts individuals. • Assist program participants in accessing and maintaining resources for independent community living. • Collaborate with clients to create a service and rehabilitation plan that meets their needs. • Provide ongoing support and monitoring to ensure services are effective. • Conduct individual sessions to help clients address challenges and set goals. • Advocate for clients to access necessary services and support. • Coordinate with families to involve them in the rehabilitation process. • Evaluate services to make improvements and enhance outcomes. • Maintain regular contact with program participants throughout their service journey. • Coordinate access to housing, social services, and other community resources. • Assist clients in navigating financial entitlements for income and healthcare coverage. • Uphold client rights and advocate on their behalf. • Facilitate access to vocational rehabilitation services. • Develop supports for employed program participants. • Assist clients with daily living skills and independence. • Monitor and assess client behavior and functioning to support treatment planning. • Connect clients with community services and resources. • Help clients learn appropriate social skills and behaviors. • Assist clients in maintaining connections with family members and support systems. • Collaborate with multidisciplinary teams to create individualized care plans. • Conduct functional assessments to inform service planning. • Participate in organizational processes such as quality assurance and health/safety activities. • Conduct regular check-ins with assigned caseload contacts. • Facilitate individual, family, and group therapy sessions.Level of Education/Training: Master’s Degree in a health-related humanservice field.
