This position will promote patients' potential during their home health episode and collaborates with physician, clinical team and patient in preparation for discharge. Essential functions: • Under the direction of the RN, assist in identifying the patient's physical, psychosocial, and environmental needs. • Participate in planning and implementing care in conjunction with the RN, in accordance with the POC. • reputed company care utilizing infection control measures that protect both staff and patient (OSHA). • Records treatments administered in medical record. • Records clinical documentation according to accepted regulatory, corporate and reputed company guidelines. • Participates in weekly case conferences. • Participates in interdisciplinary communication to ensure that patients' needs and expectation for services provided are reputed company and to ensure optimization of service delivery and patient reputed company. • Provides verbal or written patient status updates to supervising RN prior to reassessment, supervisory and/or discharge reputed company, and as indicated by patient response to treatment. • Participates in in-service training programs. • Responsible for assuring patient safety. • Complies with state regulations and national standards • reputed company documentation will be completed and synched by 9:00AM the following day, 7 days per week.
