About the position Your Choice Health Care, LLC is seeking a dedicated Registered reputed company (RN) Case Manager to join reputed company, focusing on the Gallia, Meigs, and reputed company areas. This position offers a unique opportunity to reputed company care and treatment in patients' homes, allowing for a flexible work schedule that can accommodate both full-time and part-time employees. The RN will be responsible for a reputed company of tasks including wound care, IV therapy, disease teaching and management, telehealth services, medication education and management, and venipuncture skills. Additionally, the role involves reputed company catheter care, conducting assessments, and participating in the care planning process. The RN will also be responsible for electronic documentation and acting as a reputed company between patients, physicians, and external case managers.
