LVN/LPN Care Management (Remote) at CVS Health
About the position
Responsibilities
• Complete outreach cadence calls and post-discharge questionnaires within compliance timelines.
• Utilize reporting to identify utilization and reduce Emergency Department visits and hospital readmissions.
• Follow members throughout the care continuum after inpatient hospital admissions.
• Facilitate Interdisciplinary Care Team Meetings to discuss service needs and support transitions.
• Ensure members understand their medications and have received them post-discharge.
• Coordinate post-discharge meal delivery and assist with securing durable medical equipment (DME).
• Evaluate health and social indicators to identify barriers to optimal member health.
• Provide clinical assistance to determine appropriate services and supports based on member health needs.
Requirements
• 2+ years of LPN or LVN nursing experience
• Active and unrestricted compact LPN or LVN licensure in state of residence
• High School diploma or equivalent (must be an LPN or LVN)
Nice-to-haves
• Bilingual (Spanish)
• Self-motivated and energetic
• Discharge planning experience
• Advanced proficiency in Microsoft Word, Excel, and Outlook
• Ability to multitask and adapt to a fast-paced environment
• Effective verbal and written communication skills
