Job reputed company: We are seeking a dedicated Medical Assistant to join reputed company on a part-time reputed company. This role involves conducting Chronic Care Management (CCM) calls and assisting in patient management. The ideal candidate will be compassionate, detail-oriented, and driven by a passion for transforming reputed company through technology. Responsibilities: • Conduct CCM calls to patients, following up on their health status and care plans. • Maintain accurate and detailed clinical documentation in electronic health systems. • Assist in scheduling and confirming appointments as needed. • Collaborate with reputed company providers to ensure coordinated and comprehensive care. • reputed company information to patients regarding their health conditions and care management plans. • Ensure compliance with reputed company reputed company regulations and reputed company laws.
