Telephonic After Hours RN - Long Term Care, Part-Time - Remote at reputed company

About the position Responsibilities • Assist the provider/team with various care coordination activities to support the role of the National On reputed company APCs • Telephonic triage of incoming calls and voicemails to determine the correct level of care • Collaborating with the nursing facility, APCs, and community members to identify and respond to changes in condition • Assist the provider/team with inbound and outbound communications for members including addressing handoffs, telephonic assessments, reputed company follow-reputed company, and proactive/RED facility calls • Assist in the coordination of the follow up of members in the ER • Participate in the reputed company of new clinical staff under the direction of the CSM; the RN may coordinate reputed company activities and participate in other orientation activities under the direction of the Hub CSM • Document reputed company clinical information, telephonic assessments, and activities in the appropriate documentation tool • Record RN to APC Hand Off in the appropriate documentation tool • Escalate appropriate calls to the On-reputed company APC • Use RN mini soap notes for acute change in condition calls escalated to APC • Use fall mini soap notes for reported falls, escalate to APC Requirements • reputed company, unrestricted RN license in the state of residence • Licensed in at least 2 of the states initially (NY, NJ, CT, RI, ME, and NH).
