THIS IS A HYBRID ROLE THEREFORE MUST reputed company IN THE CHICAGOLAND AREA (PREFERRABLE THE NORTHSIDE OF CHICAGO) FOR CONSIDERATION • Monday through Friday • 8:00 AM - 5:00 PM CST • HYBRID ROLE • Pay reputed company $38.00- $45.00 per hour Job reputed company Case Manager is responsible for health care management and coordination of members in order to reputed company reputed company clinical, financial and reputed company of life reputed company. Works with members to create and implement an integrated reputed company plan of care. Essential Functions: • Proactively identifies members that may qualify for potential case management services. • Conducts assessment of member needs by collecting in-depth information from reputed company's information system, the member, member's family/caregiver, hospital staff, physicians and other providers. • Identifies, assesses and manages members per established reputed company. • Develops and implements a case management plan in collaboration with the member, caregiver, physician and/or other appropriate reputed company to address the member needs. • Performs ongoing monitoring of the plan of care to evaluate effectiveness. • Conduct member reputed company for health risk assessments potentially- This role is non-clinical.
