Join KangarooHealth as a Remote Patient Monitoring Specialist LPN/LVN, where empathy, adaptability, and clinical excellence come together to deliver exceptional patient care. As a licensed LPN or LVN in California with a compact state license, you will play a vital role in supporting patients with chronic conditions, such as Hypertension, Diabetes, Heart Failure, and COPD, through our innovative digital platform.

In this full-time, fully remote position, you will conduct daily outreach to patients, review their vitals, provide personalized health coaching, and support their use of connected devices. Your expertise will help patients feel seen, heard, and cared for between visits, while your ability to troubleshoot issues and escalate concerns to providers will ensure seamless care coordination.

Requirements include:

  • Active LPN or LVN license in California, with multi-state compact privileges
  • Technical fluency, with experience working in multiple platforms and systems, such as G-Suite, Microsoft Office, Hubspot, Slack, Zoom, Epic, ECW, Athena, and more
  • 5+ years of clinical experience in hospital, family, or geriatric care, with 5+ years of EMR experience and the ability to learn new platforms quickly
  • 1+ year of experience working remotely and engaging patients by phone, with a strong understanding of RPM/CCM codes and documentation requirements
  • Bilingual fluency in Russian or Spanish is preferred, with comfort using video conferencing and digital workflows
  • Reliable internet, computer, and a private, HIPAA-compliant workspace, with availability to work 40 hours per week aligned with client clinic hours

Key functions of this role include:

  • Outreach and follow-up for remote patient monitoring and vitals tracking
  • Medication reconciliation and lifestyle coaching based on care plans
  • Documentation and escalation to providers as needed
  • Assisting patients with device setup and troubleshooting
  • Maintaining compliance with HIPAA and company protocols
  • Supporting onboarding and training of new team members

Compensation and benefits include:

  • $55,000-$65,000 total compensation, with base pay and performance-based bonuses
