We're seeking a compassionate and experienced professional to join our team as a Field Health Social Services Coordinator. In this role, you'll assess, plan, and implement care strategies that are tailored to each patient's unique needs. You'll also identify and initiate referrals for social service programs, manage care plans, and advocate for patients and families as needed.
Primary Responsibilities:
- Assess, plan, and implement individualized care strategies for patients
- Identify and initiate referrals for social service programs, including financial, psychosocial, community, and state supportive services
- Manage care plans and communicate with stakeholders to ensure quality, coordinated care
- Advocate for patients and families to ensure their needs and choices are fully represented and supported

Requirements:
- RN, LPC, LBSW, or LMSW license in the state of Texas
- 2+ years of experience working with people with Intellectual and Developmental Disabilities (IDD)
- 2+ years of experience in a community health setting or healthcare role
- Intermediate proficiency in MS Word, Excel, and Microsoft Outlook
- Reliable transportation and ability to travel up to 75% of the time in assigned region
- Access to a designated quiet workspace in your home with high-speed internet connection
Preferred Qualifications:
- 1+ year of experience with long-term care services and support, Medicaid, or Medicare
- Knowledge of federal and state requirements, including home and community-based settings regulations
- Reside within commutable distance of assigned duties
