Join Our Team: Part-Time Case Manager (Hybrid Remote Opportunity)

As a hybrid remote role, you'll enjoy the flexibility of working from home with occasional travel to program sites and partner locations in Minnesota. Job Summary: As a Case Manager, you will be responsible for coordinating and managing services provided to our participants from initial engagement to job placement. Your expertise will ensure participants receive comprehensive and effective support, empowering them to achieve their goals. This role involves assessing participant needs, developing and implementing care plans, and connecting participants with appropriate resources and services while tracking progress in our program tracking system.
Key Responsibilities:
- Participant Assessment: Conduct initial assessments and ongoing evaluations of participants' needs, strengths, and limitations, identifying immediate and long-term needs.
- Care Planning: Develop individualized plans based on assessments, participant goals, and available resources, setting measurable goals and objectives for participant progress.
- Coordination of Services: Connect participants with necessary support services or community resources, coordinating with other professionals and agencies involved in the participant's plan.
- Monitoring and Evaluation: Regularly review and update individual development plans, monitoring participant outcomes and adjusting services as necessary.
- Documentation: Maintain accurate and up-to-date records of participant interactions, assessments, care plans, and progress, ensuring all documentation meets legal and organizational standards.
- Compliance: Ensure compliance with relevant laws, regulations, and organizational policies, staying informed about changes in laws and best practices related to case management.
Qualifications:
- Bachelor's degree in social work, psychology, or a related field (preferred but not required based on experience)
- 3 years of experience in case management, social services, or a related field
- Strong interpersonal and communication skills
- Ability to work independently and as part of a team
- Excellent organizational and problem-solving skills
- Proficiency in using customer database and CRM systems
Working Conditions:
As an independent contractor, you will work remotely with flexible hours, determining your schedule to meet project deadlines and deliverables. Occasional travel to program sites and partner locations in Minnesota may be required.
