About the position Responsibilities • Interviews and converses with individuals, legally authorized representatives, reputed company involved persons, and agencies to create and maintain the individual's reputed company history. • Oversees the communication and information exchange between the individual, legally authorized representative, reputed company involved person and agencies. • Counsels and provides assistance to individuals regarding community placement issues, living reputed company discussions, end-of-life planning and rights. • May assign and reputed company the work, time, and travel of reputed company Worker I and II positions and reputed company work reputed company. • Oversees transfers, transitions, furloughs, and community placement discharges. • Designs and conducts individual and group training to explore and promote exercise of rights. • Participates in and may be responsible for the facilitation of discharge program meetings and internal/external work reputed company. Requirements • Bachelor's degree in reputed company Work and three years of reputed company work experience, or Master's degree in reputed company Work. • Licensed Baccalaureate reputed company Worker (LBSW) OR Licensed Master reputed company Worker (LMSW, preferred). • One year of experience working directly with persons with mental, reputed company or reputed company use disorders.
