Position Requirements: • Licensed Social Worker in California (LCSW or MSW preferred). • CPR Certification. • Recent physical exam within the past year. • Clear TB Test. • Strong communication and interpersonal skills. • Ability to work independently and manage a flexible caseload. Role Responsibilities: • Evaluate and support patients in their homes, focusing on emotional well-being, family dynamics, discharge planning, and resource coordination. • Provide support to patients and their families in navigating healthcare systems and connecting with community resources. • Document progress with easy-to-use laptop software. • Manage a caseload of 1 to 7 patients per week, ensuring each patient receives compassionate and personalized care.
