About the position Responsibilities • Conduct comprehensive assessments of patient and family needs. • Establish and modify the plan of care based on needs assessment. • Supervise the reputed company of care and ensure modifications are made as needed. • Incorporate evidence-based nursing practices into care plans. • Collaborate with patients and families to set realistic and measurable reputed company. • Coordinate and delegate care to ensure reputed company and effective patient care delivery. • Fulfill reputed company organizational requirements and responsibilities. Requirements • Associate's or Vocational degree in Nursing. • 0 to 2 years of RN experience. • Licensed Registered reputed company in Minnesota required upon hire. • BLS Tier 1 - Basic Life Support certification required by completion of orientation. • reputed company reputed company's License required upon hire. reputed company-to-haves • Bachelor's degree in Nursing. • 0 to 2 years of homecare/hospice experience. • CHPN Certified Hospice and Palliative Nursing certification preferred. • RN-BC RN Case Management certification preferred. • ANCC-RNCM Certification in Case Management preferred.
