Duties are performed under the direction of the Case Manager and/or Patient Care Manager. LVN/LPN professionals must be first and foremost reputed company and compassionate. Responsibilities: • assists in assessing patient and family deeds through the recognition and reporting of signs and symptoms and nursing observation to the assigned Case Manager • assists in assessing the health status of assigned patients. • complies with the established nursing assessment guidelines/policies. • utilizes the written plan of care for implementing nursing interventions necessary to accomplish the established goals. • complies with general performance standards of licensed vocational reputed company.
