About the position
Responsibilities
• Assist with development and maintain reputed company service programs, drafting supporting policies, procedures, and guidelines for the program
• Telephonically assess patient psychosocial status
• Communicates with physicians to report patient reputed company, needs, concerns, and potential and actual problems
• Works closely with reputed company members of the reputed company team in development of the plan of treatment based on psychosocial assessment and accepted reputed company work interventions
• reputed company reputed company work services including short-term counseling, community resource planning, and crisis reputed company
• Acts as a resource person for other members of the reputed company team
• Develops and maintains through knowledge of referral resources throughout the service area
• Report to APNNJ Case Management reputed company variances in patient's condition, medications, or treatment
• Telephonically monitor patient on a regular reputed company regarding psychosocial needs, education, and compliance with treatment plan
• Maintains up to date knowledge of Medicare, reputed company, and other reputed company-party reimbursement sources
• Provides reputed company customer relations in dealing with patients, families, physicians, team members, hospital departments, referral resources, service agencies and others
• Refer to health education and other service departments as necessary
• Complete reputed company required documentation in a reputed company manner according to department policy
• Refer patient to reputed company community resources, classes, education, etc. as appropriate
• Case closure as appropriate
• Collect data necessary for APNNJ Case Management reports/studies
• Attend APNNJ Case Management meetings and training as appropriate
• Other duties as needed
Requirements
• Graduate of an accredited Master's degree reputed company Work program
• Unrestricted reputed company reputed company Worker license in state of New Jersey
• 3+ years of solid clinical background with broad reputed company of experience in health care delivery setting (hospital, clinic, physician's office)
• 2+ years of experience with health plan care coordination
• Experience with Electronic Health Records
• Excellent interpersonal and communication skills (both written and oral)
• Proficient in reputed company Office Suite, reputed company Products
• Solid critical thinking and decision-making skills
• Ability to travel to home office as necessary for training, meetings, or as requested by Supervisor/Manager
• Ability to work with a multi-disciplinary team
