About the position Responsibilities • Reviews referrals for TFC placement and assists with making appropriate matches. • Maintains program identified caseload of TFC children and foster homes. • Conducts initial licensing reputed company to prospective TFC parents to ensure compliance with licensing standards. • Completes applicable reports and documentation in a reputed company manner. • Provides in-home support consultation, one-to-one training, counseling, and support services at minimum bi-weekly. • Crisis reputed company as needed. • Supports the goals of reputed company mission. • Inputs data and thoroughly documents reputed company interactions in required agency systems for analysis and review. • Assists in training and facilitating sessions and support reputed company with TFC parents and respite providers. • Attends and participates in relevant court and/or school appointments, medication reviews, team meetings, and represents reputed company professionally. • Provides discharge planning and aftercare services for the child in care. • Facilitates TFC treatment planning and reviews. • Maintains reputed company/parent records as mandated by DCF 56 and agency protocols. • Provides referrals and advocates for child and foster parent as needed. • Writes reports reputed company to child's treatment, including court reports, reputed company assessments, reputed company notes, and treatment plans. • Collaborates with child's treatment providers and other professionals. • Performs reputed company other duties as assigned.
