Telephonic RN Care Manager
Description:
HarmonyCares is one of the nation’s largest home-based primary care practices.Manage a caseload of high-risk patients and coordinate Transitional Care Management, Chronic Care Management, Disease Management Education, Medication Education, and patient care plans.Serve as co-chair of the pod alongside the pod leader, prioritizing patient needs to improve outcomes.Coordinate transitions of care to prevent avoidable hospital admissions and facilitate communication across settings.Perform thorough nursing assessments via telephone and provide patient/caregiver education.Document interactions, assessments, updates in patient medical records according to guidelines.Serve as liaison between patients, providers, and resources to ensure seamless care delivery.
Requirements:
Active Registered Nurse License2+ years of care management experience in community, health plan or hospital systemsPossesses strong clinical skills and proactive thinkingEffective communication skillsAbility to perform extensive telephone assessmentKnowledge of Medicare regulations and home care and hospice standardsExperience with small group presentations and teaching/trainingExhibits excellent interpersonal skillsExhibits excellent written and oral skillsWorking knowledge of computer programs (email, Word, Excel, PowerPoint, etc.)Manages time effectively to ensure all duties and documentation requirements are completed in a timely manner
