About the position Responsibilities • Oversee and manage clinical team processes to ensure high performance. • Ensure effective functioning of care management and care coordination activities. • Assess, plan, implement, coordinate, monitor, and evaluate care services. • Develop and support clinical team members in their roles. • Communicate effectively with members who have complex medical or social needs. Requirements • Active unrestricted Virginia State License in applicable functional area (RN or BH). • 5+ years in clinical area of expertise. • 2+ years supervisory/managerial experience. • 3+ years care management experience. • 3+ years of experience with personal computer, keyboard navigation, and MS Office Suite applications. • Experience working in health care delivery systems (electronic documentation platform). • Ability to travel for state-required meetings and trainings (up to 20% travel estimated). Nice-to-haves • Medicaid experience - LTSS highly preferred. • Knowledge of regulations, standards, and policies related to medical management. • Certified Case Manager (CCM) certification.
