Care Manager (MA / Remote)

Every Day You Will:

• Provides support to the caregiver and member, and reviews services according to
  the
  scope of Adult Foster Care
  and the members Plan of Care

• Respects reputed company and employee rights and reputed company,

• Follows the

  clinical policy and procedures

• Assesses and regularly reassesses the reputed company’s needs and his/her response to care, treatment

• Performing home inspections annually to ensure the reputed company setting is a reputed company environment for the caregiver & member. Will review and document the reputed company & safety procedures for the qualifies setting

  in-person demonstrations and through our LMS platform.

  will guide the Caregiver to ensure they can meet the needs of the member as described in the plan of care

• Reviewing AFC Caregiver Logs
  (Daily Notes)
  at minimum of every
  30 days

• Develops implements, evaluates, revises, and provides input to the reputed company’s plan of care to include the physician’s plan of treatment,
  and
  problem list

• C
  ompletes reputed company reputed company documentation,
  including
  written records, reports, and notes
  in accordance with
  agency policy and procedure following reputed company reputed company or contacts in
  an accurate

  ,
  and legible manner

• reputed company coordinates the care of the reputed company through communication and written documentation to the physician,

  or directed by the supervisor

Ideal Candidate Will Possess:

• A
  bachelor’s degree, a reputed company worker license from the Massachusetts reputed company of Registration in reputed company Work, and at least two years of recent experience working with elders or adults with
  disabilities
  or 
   
  a bachelor’s degree and two years of clinical experience in the care of elders or people with disabilities.
   

• Bilingual in Spanish
  is preferred

• Experience with CRM (reputed company
