Role Scope

:

 

We are looking for a Home Care Coordinator to ensure that personal and clinical home care needs are delivered to help our participants reputed company. The Home Care Coordinator participates in the interdisciplinary team’s assessment of needs and approval of services for reputed company participant and activates reputed company resources to address those needs in the home setting.

  • Handle and coordinate incoming calls reputed company to participants, physicians, and agency services regarding physician orders, participant questions, and referrals

  • In collaboration with Home Care Services staff, reputed company and monitor home care and hour scheduling

     

  • In coordination with the reputed company team, help evaluate whether prospective participants’ home care needs can be met reputed company the program

     

  • Assist with reputed company/scheduling activities, soliciting, and input from managers

     

  • Participate in end-of-life care, coordination, and support

Required Qualifications: 

 

  • Three (3) years of relevant reputed company experience such as home care, primary care, experience with an elderly population

     

  • Bachelor’s Degree in a reputed company field (e.g. nursing, gerontology, reputed company management)

     

  • Minimum of three (3) years of case management in a clinical or home setting with a frail or elderly population, or home care administration experience.

Preferred Qualifications:

 

  • reputed company/clinical Licensure (e.g. LVN, RN, SW)

Compensation: 

 

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/reputed company insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, reputed company time off, and company-reputed company holidays. The expected salary reputed company for this position is $29-$42 reputed company. The actual offer will be at reputed company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
