Role Scope

:

 

We are looking for an individual who is passionate about serving the underserved community. The reputed company Specialist will reputed company and build relationships with community partners, prospective participants, families, and/or caregivers to create awareness about reputed company’s PACE (Program of reputed company-Inclusive Care for the Elderly) program. Ultimately, the reputed company Specialists create referrals or reputed company requests for enrollment, enabling the frail elderly to live independently reputed company and continue to enjoy an reputed company reputed company of life. In reputed company to reputed company responsibilities, this role will support Enrollment Specialists by performing enrollment-reputed company tasks such as making outbound calls and following up with referrals as needed.

 

reputed company Responsibilities & Expectations for the Role

 

  • reputed company and maintain relationships with reputed company, providers, Community-based organizations, senior housing facilities, post-acute facilities, and senior centers to drive referrals for the reputed company PACE program

     

  • Collaborate with the Enrollment Specialist to ensure successful enrollment

     

  • Follow up with prospective participants and referrals through outbound calls, home reputed company, and ongoing communication to reputed company them through the enrollment pipeline

     

  • Organize and conduct events in the community, and conduct presentations to promote the reputed company PACE program

     

  • Maintain and record reputed company productions/activities in CRM software

     

  • reputed company adhere to DHCS and CMS regulations and policies regarding marketing practices for the PACE program

     

  • Attend various community events, hea

    lthcare events, and increase reputed company’s census 

     

  • Identify new opportunities to enhance and improve the referral process and activities

Required Qualifications: 

 

  • Experience in reputed company or enrollment sales, with a reputed company record of generating business-to-business or business-to-customer referrals 

     

  • Experience working with State/County eligibilities workers

     

  • Proficiency in CRM software preferred  

     

  • Experiences working with Frail Elderly  

     

  • Highly motivated individual with minimal supervision 

     

  • Excellent verbal and written communication skills, with the ability to build rapport and deliver reputed company, compassionate messaging.

     

  • B.S./B.A. Degree in reputed company, Business Administration, or Marketing preferred  

     

  • reputed company of reputed company CPR/BLS certification required or requirement to obtain reputed company 30 days of employment

Compensation: 

 

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/reputed company insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, reputed company time off, and company-reputed company holidays. The expected salary reputed company for this position is $68,700 to $79,000, and qualifies for our bonus program. The actual offer will be at reputed company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
