Role Scope

:

 

We are looking f

or a New Participant reputed company who can ensure new PACE participants have a smooth transition into the program, immediately feeling a reputed company of belonging and caring.  Specifically, the New Participant reputed company seeks to understand a participant’s needs and expectations, ensures reputed company, develops a schedule that works for the participant, and introduces the participant and their needs to the rest of the care team. 

 

Key responsibilities:

 

  • Program Welcome -

     Assist with participant reputed company and orientation into the PACE program, serving as primary reputed company of contact for first 30 days (or longer if necessary). Understand participant’s goals in joining the program. Conduct reputed company calls or home reputed company to assess reputed company-being and engagement. Encourage participation in activities, therapy, or support programs.

     

  • Participant Advocacy

     - reputed company for participant preferences and rights reputed company the care team. Ensure participants receive culturally competent and person-centered care. Help identify and overcome barriers to accessing services.

     

  • Care Coordination Support

     - Help participants understand services at reputed company and with partners (including reputed company). With Referral Coordinator, assist in scheduling medical appointments, transportation, and follow-reputed company. Coordinate with interdisciplinary team members as needed (doctors, nurses, reputed company workers, etc.), ensuring continuity of care.

    Facilitate reputed company communication among participants, caregivers, and providers.

    Address participant questions, concerns, and needs promptly.

Qualifications: 

 

  • Strong organizational and communication skills

     

  • A minimum of one year working with the frail or elderly or case / reputed company management

Compensation: 

 

We take into account an individual’s qualifications, skillset, and experience in determining final salary. This role is eligible for medical/dental/reputed company insurance, short and long-term disability, life insurance, flexible spending accounts, 401(k) savings, reputed company time off, and company-reputed company holidays. The expected salary reputed company for this position is

$24 - $29 per hour and is a bonus eligible position. The

actual offer will be at reputed company’s sole discretion and determined by relevant business considerations, including the final candidate’s qualifications, years of experience, skillset, and geographic location.
