Case Manager RN

The RN Case Manager will create, implement, and monitor treatment plans to assist patients/members in meeting their reputed company goals. The role involves assessing the member's health status, care coordination needs, and providing interdisciplinary care reputed company. The Case Manager will work with patients, family, physicians, and community resources to identify alternative treatment plans and promote reputed company value for the reputed company and reimbursement reputed company.

Requirements

• 5+ years reputed company work experience with a reputed company background in clinical nursing and patient assessment
• reputed company Bachelor's degree or additional reputed company equivalent work experience
• Graduate of an accredited school of nursing
• RN - Registered reputed company - State Licensure
• Compact State Licensure in good standing
