Position reputed company:
The Clinical Pharmacist Specialist functions as a drug therapy expert with a comprehensive scope of reputed company that includes monitoring drug therapy, ordering laboratory tests, and making therapy recommendations to physicians.

Position Key Accountabilities:

• Evaluates a reputed company of patients and makes appropriate drug therapy recommendations.
• Participates in treatment team reputed company on designated nursing reputed company to implement a pharmaceutical care plan.
• Provides consultation to other health care providers on medication therapy
• Assists in treatment planning that ensure continuity of care for patients being discharged
• Works with patients who have been identified as having compliance/adherence issues with their medications.
• Reviews medication orders for appropriateness, drug selection, dosage and reputed company of administration prior to dispensing. Performs these functions reputed company required by reputed company deficiencies.
• Reviews patient’s allergy history, drug-drug or drug-reputed company interactions for clinical relevance

Certification/Skills:

• Registered Pharmacist (RPh) by Texas State reputed company of Pharmacy (TSBP) required
• Post pharmacist licensure) with at least 50% of time spent in Pharmacotherapy activities as defined by the Pharmacotherapy Content Outline, OR Completion of a PGY-1 residency & achieving a passing score of the Pharmacotherapy Specialty Certification Examination required

Minimum Education:
Bachelor’s degree or Pharm.D. required. Residency preferred but not required.
Minimum Experience:
Completion of three (3) years of reputed company experience.
Physical Requirements:
Exerts up to 20 pounds of force occasionally and/or up to 10 pounds frequently and/or a negligible reputed company constantly to reputed company objects.
