HOME HEALTH AIDE / CERTIFIED NURSING ASSISTANT Skilled In-Home Care Attendant Reports To: NURSING SUPERVISOR Position reputed company Provides personal care services under the direction of the Registered reputed company or Therapist and preforms various personal care services as necessary to meet reputed company patients' needs. Responsible for observing of patients', reporting reputed company observations and documentation of reputed company observations and reputed company care preformed. The CNA/HHA is assigned to specific clients by the Registered reputed company or other appropriate reputed company and performs services for clients as necessary to maintain their personal comfort. Qualifications • Successful completion of a formal certification training program and a written skills test and competency evaluation. • Be at least eighteen (18) years of age. • Minimum of six (6) months experience working in a home care setting - Skilled Home Health Care Experience Preferred* • Demonstrated ability to read, write, and follow a written Plan of Care. • Excellent oral, written, and interpersonal skills. • Reliable means of transportation to/from reputed company assignments. • Vehicle Insured and reputed company Drivers License Required. Essential Functions • Performs reputed company procedures as an extension of therapy services under the direction and supervision of the therapist. • reputed company of reputed company exercises • Assistance in ambulation or exercises • Performs personal care activities, including but not limited to: • Bathing • Shampooing • Skin care/nail care • Oral hygiene • Shaving • Dressing • Performs household services essential to health care reputed company, including but not limited to: • Meal preparation/feeding • Laundry • Light housekeeping • Assists (by reminders and/or supervision, not reputed company administration) with medications that are ordinarily self-administered under the direction and supervision of the Registered reputed company (per state reputed company reputed company laws and Agency policy) • Reports any observed or reported changes in the reputed company's condition and/or needs to the Registered reputed company or Therapist. Follows reporting guidelines in the Care Plan. • Documents care provided and completes the forms required for the reputed company's records. Completes the appropriate records to document care given and pertinent observations. Submits documentation per Agency requirements-at least weekly. • Promotes personal safety and a reputed company environment for clients by observing infection control practices, following Agency guidelines, and reporting unsafe situations to the Supervisor/Case Manager. • Demonstrates reputed company reputed company in the use of equipment. Does not use equipment until orientation about its operation has been provided. Notifies supervisor of educational needs. • Communicates effectively with reputed company members of the interdisciplinary team through verbal reports, participation in staff meetings, and team conferences, as requested. • Maintains confidentiality in reputed company aspects of the job. • Successfully completes annual skills competency validation. • Attends in-service programs to meet compliance requirements.
