Job Summary

As a Field Clinical Educator with CenterWell Home Health, you will be responsible for teaching, mentoring, and coaching clinicians in the home health environment. You will instruct and counsel clinicians while acting as a role model, leader, influencer, advocate, evaluator, and socialization agent. Your primary goal will be to ensure the growth and retention of new hires, while promoting a culture of learning and support.

Key Responsibilities

  • Teach, mentor, and coach clinicians in the home health environment, focusing on the application of knowledge obtained from training.
  • Demonstrate competence in performance of clinical skills, including admission of patients, assessment, and development of care plans, case management, coordination of care, and other clinical care duties.
  • Coordinate, oversee, and supervise the work of new hires and other patient-facing clinicians in the classroom and field environment.
  • Model regulatory compliance and standards, ensuring that clinicians understand and adhere to established guidelines.
  • Provide oversight and education to ensure effective collaboration with healthcare providers, in accordance with physician orders and plans of care.
  • Act as a liaison and maintain communication with new hires, leadership, peer mentors, and the Home Health Learning & Development team.
  • Ensure execution of the orientation experience, including:
    • Orientation toolkit
    • Competency assessments
    • State-specific requirements as needed
  • Continually work to create a culture of learning and support for clinicians as they gain confidence and competence in their respective roles.
  • Responsible for established Key Performance Indicators (KPIs).

Requirements and Qualifications

Essential Qualifications:

  • Diploma or Associate's degree in Nursing.
  • Unrestricted RN license in the state assigned.
  • Current CPR certification.
  • Demonstrates strong leadership skills and the ability to function with minimal supervision.
  • Displays effective communication, interpersonal skills, and sound decision-making skills.
  • Possesses effective teaching skills and a desire to mentor and develop clinicians.
  • Ability to assess the learning needs and skills of new clinicians/students and provide immediate and ongoing feedback.
  • Ability to create a nurturing and supportive environment conducive to learning, using adult learning principles in the classroom and/or field.
  • Ability to validate the performance of new clinicians/students and provide effective feedback to clinicians/students and leaders in the location.
  • Working knowledge of healthcare laws and regulations, including state/local specific.
  • Competent in the delivery of patient care consistent with evidence-based practice and regulatory requirements.
  • Demonstrates proficiency in OASIS accuracy (75%).

Preferred Qualifications:

  • Bachelor's or Master's degree in Nursing.
  • HCHB experience.
  • OASIS certification.
  • Prior nurse leadership or educator experience.
