Key Responsibilities
- Conduct routine physical examinations and formulate medical, educational, and psychosocial plans to treat patients in their homes
- Develop plans for further evaluation of health needs, including referrals to other health team members or specialists
- Provide appropriate treatment for problems within the scope of practice, as designated by protocols, and identify problems outside of scope and refer appropriately to physician
- Collaborate with the practice physician(s) and care team members to develop comprehensive care plans for patients
- Utilize EPIC, our state-of-the-art clinical and informatics technology, to promote improved patient outcomes, streamlined continuity of care, and superior provider satisfaction
Qualifications and Requirements
To be successful in this role, you will need to possess the following qualifications and skills:
- Unrestricted State of Connecticut Medical License (or eligible)
- APRN (Family, Adult/Geri Nurse Practitioner) or PA certification
- At least two years of post-graduate experience as a Nurse Practitioner (APRN) or Physician Assistant (PA)
- Strong clinical skills and knowledge of geriatric medicine
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams
- Ability to work independently and as part of a team, with a strong focus on patient-centered care
Preferred Qualifications
- Experience working in home care or geriatrics
- Knowledge of EPIC or other electronic medical records systems
- Certification in gerontology or a related field
- Bilingual or multilingual skills
