Job Summary
We are looking for a Case Management Analyst to work with our team to facilitate and coordinate the case management process for our patients. The successful candidate will use their analytical and problem-solving skills to assess patient needs, develop care plans, and collaborate with healthcare professionals to ensure that our patients receive the best possible care. This is a part-time remote opportunity that offers a competitive hourly rate of $30.
Key Responsibilities
- Patient Assessment: Conduct comprehensive assessments of patient needs and qualifications using care management tools and data/information review. Prescribe a approach to case goal or potentially addressing needs by assessing patient benefit plans and available inside and outside programs/services.
- Care Planning: Facilitate and carry out assigned care plan activities and monitor care plan progress. Identify high-risk factors and service needs that might impact patient outcomes and care planning parts with suitable reference to clinical case management or emergency intervention as appropriate.
- Clinical Coordination: Collaborate with caseworkers, bosses, Clinical Chiefs, and other healthcare professionals to overcome barriers to meeting goals and objectives. Present cases at case meetings to get multidisciplinary review to achieve optimal outcomes.
- Quality of Care: Identify and heighten quality of care issues through established channels. Use discussion skills to secure suitable choices and services necessary to meet the patient's benefits and healthcare needs.
- Patient Engagement: Use influencing/inspirational interviewing skills to ensure maximum patient engagement and promote lifestyle/conduct changes to achieve optimal level of health. Provide education, information, and support to empower the patient to make ongoing autonomous clinical or potentially healthy lifestyle choices.
- Documentation: Use case management and quality management processes in compliance with regulatory and accreditation rules and company policies and procedures.
Requirements
Essential Qualifications
- Education: Four-year bachelor's degree or non-authorized master's level clinician with either degree being in behavioral health or human services preferred (psychology, social work, marriage and family therapy, counseling) or equivalent experience.
- Experience: 2+ years of experience in behavioral health or social services.
- Technical Skills: 2+ years of experience with Microsoft Office applications (Word, Excel, Outlook).
Preferred Qualifications
- Case Management Experience: Case management and discharge planning experience.
- Managed Care Experience: Managed care insight.
