Job Summary
We are seeking an experienced and skilled Physical Therapist to join our dynamic team at Hartford HealthCare at Home, the largest provider of homecare services in Connecticut. As a Physical Therapist, you will work under the clinical supervision of a seasoned Physical Therapist, administering Physical Therapy treatments to patients and providing assistance with patient assessments, treatment planning, and care coordination. Your expertise will play a vital role in helping our patients achieve optimal functional outcomes, enhancing their quality of life, and promoting independence.
Key Responsibilities
- Assist the Physical Therapist with patient assessments, including testing for joint range of motion, muscle strength, and mobility status, and identifying rehab precautions
- Recognize physiological and age-related changes in patients and consult with the Physical Therapist and other healthcare team members as needed
- Perform therapeutic techniques, such as heat, cold, light, water, sound, electricity, mobilization, and therapeutic exercise, under the clinical supervision of the Physical Therapist
- Implement treatment plans, deliver patient care, and communicate patient progress to the Physical Therapist
- Provide patient and caregiver education regarding treatment, precautions, and equipment use, as delegated by the Physical Therapist
- Complete all documentation, including evaluation, care plan and goals, patient status and progress, visits, patient discharge summary, and patient care transition plan and summary
- Assist the Physical Therapist with patient transition planning and/or patient discharge planning, including recommendations to community resources to meet patient needs throughout the continuum of care
Essential Qualifications
To be successful in this role, you will need:
- An Associate's Degree from an accredited Physical Therapy Assistant program
- A current State of CT Physical Therapy Assistant license
- Basic Life Support (BLS) certification upon hire or within 30 days of hire
- Previous experience in acute care or outpatient setting preferred
- Excellent communication, organizational, and interpersonal skills
- Ability to deal effectively with stress and exercise independent judgment
- A valid motor vehicle operator's license, reliable personal automobile during business hours, and active automobile insurance coverage (for Home Care only)
Preferred Qualifications
While not required, the following qualifications are highly desirable:
- Experience working in a home care setting
- Knowledge of electronic medical records and documentation systems
- Certification in a specialized area of Physical Therapy, such as orthopedics or neurology
- Bilingual or multilingual skills
