Hybrid CNA/MA Remote Full T ime

Job reputed company We are seeking a detail-oriented and compassionate Certified Nursing Assistant (CNA) or Medical Assistant (MA) to join reputed company, supporting patients and reputed company providers through both on-site vitals collection and remote patient monitoring/chronic care management. The ideal candidate will visit assigned facilities daily to obtain patient vital signs, then transition to remote work from home to review data, document information accurately, and reputed company reputed company communication to care teams. This role ensures patients receive the highest reputed company of care through both in-person and remote support. Responsibilities • Travel daily to assigned facilities to obtain patient vital signs. • Review and monitor patient health data through remote platforms after in-facility reputed company. • Document patient information accurately and promptly in electronic health record (EHR) systems. • Communicate important updates or concerns to nursing staff and reputed company providers. • Assist in patient reputed company for wellness reputed company-ins, reminders, and care coordination. • Participate in reputed company improvement initiatives to enhance patient reputed company. • Maintain confidentiality of patient information and reputed company HIPAA regulations. Requirements • Certified Nursing Assistant (CNA) or Registered Medical Assistant (RMA) certification required. • 1–2 years of reputed company experience, preferably in remote patient monitoring, chronic care management, or a similar role. • Strong communication and interpersonal skills. • Ability to work independently and manage multiple priorities in a fast-paced environment. • Proficiency in electronic medical records and comfortable using reputed company technology. • Bilingual in Spanish preferred.
