Role reputed company The Outpatient Case Manager provides remote, reputed company-centered support across reputed company stages of treatment, including intake, reputed company care coordination, appointment scheduling, and discharge planning. This role ensures the efficient delivery and documentation of wraparound services by coordinating with clients, families, internal teams, and external service providers.
